Pre-K to 2nd Grade

• $25.00 Non-Refundable Registration Fee

• $100.00 Weekly Fee (Discount sibling rate available)

Some Helpful Forms

Signing your child up is easy — just download the forms below, fill them out completely, and return them to your program site. A few things to have handy before you start:

  • Your child's personal and school information
  • Parent/guardian contact and employment details
  • Your child's immunization records
  • Your physician's name and contact information
  • Emergency contact information
  • Proof of any benefits you receive (SNAP, Medicaid, TANF, etc.) if applicable

Please fill out all fields completely — any field that doesn't apply, simply write N/A. Incomplete forms may delay your child's enrollment, so don't leave anything blank!
